病假条字数很少，也算不上英语作文，写个简单的例文吧(以MRSMARY是老师为例)：

Dear Mrs mary，

I beg to apply for seven days’leave of absence from the 1th to 7th of this month.Because I felt dizzy and had a fever. The doctor said I needed a week's rest for my flu.So I need to return home to have a rest.When I am better,I will do my best to catch up on my studies as soon as I get back from leave.
